OFFICIAL RULES AND REGULATIONS
$500 Gift Card Monthly Survey Sweepstakes with Raley's, Bel Air, Nob Hill Foods and Food Source:
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1. Prizes:
a. Each month (from July 2009 through June 2010), one (1) Raley's, Bel Air, or Nob Hill Foods Gift Card valued at $500 will be awarded to a grand prize winner. The monthly winner will be selected by random drawing from eligible entries received during that month only. Entries from previous months are not eligible. The monthly winner of the gift card will be subject to the terms and conditions as set forth by the provider of the gift card.
b. Odds of winning depend upon the number of eligible entries received that month.
c. Prize may be picked up at Raley's, 500 West Capitol Ave., West Sacramento, CA 95605 or at your local Raley's, Bel Air, Nob Hill Foods, or Food Source store. Any prize not claimed within thirty (30) days of winning will be forfeited by the winner. In the event that a prize is mailed to the winner, it will be with the prior written consent of the winner and winner assumes the risk of its loss. Raley's Family of Fine Stores is not responsible for the safe arrival of the prize or prize certificate.
d. There is no substitution, transfer, or cash equivalent for prizes, except that Raley's Family of Fine Stores may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses.
2. How to Enter:
a. Sweepstakes will begin on July 1, 2009 at 12:00 a.m. Pacific Time ("PT") and end on June 30, 2010 at 11:59 p.m. PT.
b. To be eligible, entries for each monthly drawing must be received between 12:00 a.m. PT the first day of the month and 11:59 p.m. PT the last day of the month (the "deadline"). For example, to be eligible for the July 2009 drawing, all entries must be received between 12:00 a.m. PT on July 1, 2009 and 11:59 p.m. on July 31, 2009.
c. Consumers register online at raleys.com or nobhill.com and may register and enter to win once per month. To enter online, you must enter your valid email address, phone number and zip code. Information on age group, ethnicity, gender and household income is voluntary and participants preferring not to provide this information should select the "Decline to State" option in the appropriate drop-down box. Participants must answer the fifteen main survey questions. Limit one internet entry per household and one internet entry per email address per month. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible for assigning email address or the domain associated with the submitted email address. Raley's, Bel Air Mart, Nob Hill General Store, Inc., and Warehouse Concepts (collectively "Sponsors") are not responsible for entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties or other technological glitches. For entry into the sweepstakes without completing the survey, mail a 3” x 5” card containing your complete name, address (including zip code), a valid email address (if available) and phone number in an envelope no larger than 4 1/8” x 9 ½“ to: Raley’s Monthly Survey Sweepstakes Entry, PO Box 15618, Sacramento, CA 95852. Mail-in entries must be postmarked before the end of the month to be included in that months drawing and received no later than the 6th day of the following month. Sponsor assumes no responsibility for incomplete, damaged, misdirected, illegible, late, stolen or lost entries. One mail-in entry per household, per month.
d. Take survey by telephone. If unable to take survey online due to lack of internet access, contact Raley's Consumer Affairs Department at 1-800-925-9989.
3. Eligibility Restrictions:
a. The sweepstakes is open to participants who are 18 years of age or older and legally reside in California or Nevada (in one of the following counties: Alameda, Alpine, Amador, Butte, Calaveras, Carson City, Churchill, Colusa, Contra Costa, Del Norte, Douglas, El Dorado, Elko, Fresno, Glenn, Humboldt CA, Humboldt NV, Lake, Lassen, Madera, Mariposa, Marin, Mendocino, Merced, Modoc, Mono, Monterey, Napa, Placer, Plumas, Sacramento, San Benito, San Francisco, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Shasta, Sierra, Siskiyou, Solano, Sonoma, Stanislaus, Sutter, Tehama, Trinity, Tuolumne, Washoe, Yolo, Yuba). Void elsewhere and where prohibited by law. Employees of Raley's, affiliated entities, their affiliated advertising agencies, participating sponsors/promotional partners and the members of their immediate families are ineligible to participate or win.
b. Only one (1) winner per month will be selected.
c. Winner must show a government-issued photo ID showing date of birth in order to pick up prize.
4. Selection of Winners:
a. Following each month of sweepstakes, the winner will be selected via random selection from all eligible entries received by the deadline. Decisions of Raley's Marketing Department with respect to the sweepstakes are final.
b. Winner will be notified via e-mail or phone on or before the 15th of each month during the duration of the sweepstakes. If winner cannot be reached within 7 (seven) days after he or she is selected, an alternate winner may be selected via random drawing from all remaining eligible entries and notified by telephone or email.
5. Conditions:
a. Payments of all federal, state and local taxes are the sole responsibility of the winner. Winner may be required to sign an IRS Form W-9 or the equivalent.
b. By participating in the sweepstakes, the winner agrees to have his or her name, voice, or likeness used in any advertising or broadcasting material relating to this sweepstakes without additional financial or other compensation, and to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Sponsors in their sole discretion reserve the right to disqualify any person tampering with the entry process, the operation of the Sponsors' Websites or is otherwise in violation of the rules. Sponsors further reserve the right to cancel, terminate or modify the sweepstakes if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
d. Sponsors reserve the right to make changes in the sweepstakes rules, including the substitution of a prize of equivalent value.
e. Failure to comply with the sweepstakes rules may result in a participant's disqualification at the sole discretion of Sponsors.
f. Sponsors are not responsible for typographical or other errors in the printing, the offering or the administration of the sweepstakes, or in the announcement of a prize.
g. Entries received via mail must be handwritten. No ink stamps or address labels allowed. No reproductions of entry blanks accepted.
h. Copies of the written Official Rules and Regulations and a list of all winners (when complete) for this sweepstakes are available online at www.raleys.com and www.nobhill.com.
6. Internet Contest Privacy Policy:
a. By use of this website and by entering the sweepstakes, you agree to Sponsors' Website Privacy Policy and to the use of your personal information as described in the Privacy Policy located at raleys.com/privacy.jsp and nobhill.com/privacy.jsp.
