The following sweepstakes is intended for viewing in California and Nevada only and shall only be construed and evaluated according to California and Nevada law.

 

1. How to Enter the Sweepstakes:

 

a. Online Entries: Enter online at www.surveymonkey.com/. Entries must be received between 10:00 a.m. on Wednesday September 16, 2009 and 11:59 p.m. on Tuesday September 22, 2009. To enter online, the entrant must enter his or her valid email address, phone number and zip code in the online entry form. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible for assigning the email address or the domain associated with the submitted email address.

 

2. Eligibility Restrictions:

 

a. The sweepstakes is open to participants who legally reside in California (in one of the following counties: Alameda, Alpine, Amador, Butte, Calaveras, Colusa, Contra Costa, Del Norte, El Dorado, Fresno, Glenn, Humboldt, Lake, Lassen, Madera, Mariposa, Marin, Mendocino, Merced, Modoc, Mono, Monterey, Napa, Placer, Plumas, Sacramento, San Benito, San Francisco, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Shasta, Sierra, Siskiyou, Solano, Sonoma, Stanislaus, Sutter, Tehama, Trinity, Tuolumne Yolo, and Yuba) and 18 years of age or older at time of entry, are eligible to enter except employees of Raley’s, Bel Air Mart, Nob Hill General Store, Inc., Warehouse Concepts dba (Food Source), their agents, affiliates, subsidiaries (collectively “Sponsor”) and members of their immediate families or persons residing in the same household are not eligible to enter or win.

 

3. Prizes:

 

a. One (1) Grand Prize Winner will receive a $75 gift card. 

b. Odds of winning depend upon the number of eligible entries received.

c. There is no substitution, transfer, or cash equivalent for prizes, except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses.

 

4. Selection of Winners:

 

a. Decisions of Raley's Marketing Department with respect to the sweepstakes are final.

b. No later than September 25, 2009 the winner will be selected via random selection from all eligible entries received by the deadline.

c. The winner will be notified by email or telephone on or after September 25, 2009. In the event the potential winner does not respond to prize notification within 5 (five) days of attempted notification or declines the prize, another winner will be randomly chosen from the remaining eligible entrants.

 

5. Conditions:

a. Limit one entry per household.
b. All federal, state and local taxes, if any, are the responsibility of the winner.
c. Except where prohibited by law: (i) entry constitutes permission for Sponsor to use winner’s name, hometown, likeness, photograph, voice and statements regarding this sweepstakes for editorial, public relations, promotional and advertising purposes in any media, including the Internet, for any purpose whatsoever on behalf of Sponsor without compensation; (ii) each potential winner will be required to complete and return an Affidavit of Eligibility, Winner’s Agreement and Release within five (5) days of notification. If winner notification is not returned or is returned as undeliverable, prize will be forfeited and another name will be drawn. By participating in the sweepstakes and winning the prize, winner releases Sponsor, its parents, affiliates, subsidiaries, and agencies and their respective directors, officers, employees and agents from any and all liability with respect to the prize won and participation in the sweepstakes. Sponsor assumes no responsibility for entries it is unable to process due to network, hardware or other technical failures; or any other reason, or incomplete, damaged, misdirected, illegible or lost entries.
d. Sponsor reserves the right, in its sole discretion, to disqualify any person tampering with the entry process or that is otherwise in violation of the rules. Sponsor further reserves the right to cancel, terminate or modify the sweepstakes if it is not capable of completion as planned, including force majeure. In the event of cancellation, Sponsor reserves the right to award the prize to eligible, non-suspect, entries received prior to date of cancellation. Any conduct that impedes the integrity of the sweepstakes is prohibited.
e. Sponsor reserves the right to make changes in the rules of the sweepstakes, including the substitution of a prize of comparable value.
f. Failure to comply with these Official Rules may result in a participant's disqualification, at the sole discretion of Sponsor. Participants agree that Sponsor is not liable for injuries, losses or damages of any kind arising from participation in this sweepstakes or acceptance, possession or use of prizes.
g. Sponsor is not responsible for typographical or other errors in the printing, the offering or the administration of the sweepstakes, or in the announcement of a prize.
